60歳以上のスペイン人女性110名を、ピラティス群55名と無介入の対照群55名に無作為に割り付け、12週間のプログラムを行った研究があります。
結果には、はっきりした線が引かれました。

この分離は偶然ではありません。そしてこの分野には、もっと大きな問題があります。最大規模で最も厳密な試験は、何の効果も見つけられませんでした。

介入期間は18か月。6か月時点で97.1%、18か月時点で81.2%が完了しました。
「6か月時点で、マインドフルネス訓練にも運動にも、エピソード記憶や実行機能への有意な効果はなかった。副次エンドポイントである18か月時点でも、介入効果はなかった。」
「あらかじめ規定された5つの副次アウトカム(海馬体積、背外側前頭前野の厚さと表面積、機能的認知能力、自己申告の認知的不安)のいずれも、有意な改善を示さなかった。」
結論——「主観的な認知の不安を持つ高齢者において、これらの介入が認知を改善するために用いられることを、本知見は支持しない。」
運動量は週300分以上、期間は18か月。これ以上の条件を整えることは難しい水準の試験です。それでも何も出ませんでした。
統合解析では、心身系運動が上位に来る
一方、多数の小規模研究を集めた解析では、結果が違います。

効果量は 0.155 から 0.48 まで幅があります。いずれも「小さい」から「中程度」の範囲です。
下位領域まで分けた25試験の解析では、さらに細かい違いが出ています。ワーキングメモリ(g = 0.127、I² = 0%)、認知的柔軟性(0.511、I² = 89.08%)、抑制制御(0.136、I² = 0%)は有意でしたが、計画立案と意味性言語流暢性には有意な効果がありませんでした。そして心身系運動が有意に改善したのは、ワーキングメモリのみでした(Xiong ら, 2020, International Journal of Nursing Studies)。

誰に効くのか、という問題

ここには、注意すべき逆説があります。効果が最も小さいのは、75歳を超えた人と、軽度認知障害のある人——つまり最も認知機能の低下が心配な層です。
そして期間が長いほど効果が小さいという結果も出ています。これは通常、出版バイアスや小規模研究効果を疑うべき所見です。
軽度認知障害に絞った24論文2,278名の解析では、心身系運動だけが「切り替え」に有意な効果を示したと報告されています。ワーキングメモリについては有酸素運動と同程度でした(Lin ら, 2022, Ageing Research Reviews)。
神経変性疾患を対象とした21試験の解析でも、MMSE で MD 0.65、MoCA で MD 0.87 という改善が報告されています(Liu ら, 2026, Frontiers in Public Health)。MMSE で 0.65点という変化が、生活のうえでどれだけ意味を持つかは、別に考える必要があります。
ピラティスに特化した研究
ピラティスそのものを調べた研究は、いずれも小規模です。

52名の試験は示唆的です。ストループ課題でも言語流暢性でも、ピラティス群と有酸素運動群のあいだに差はありませんでした。両群とも前後では大きく改善しており(効果量 0.54〜1.45)、ピラティス群でのみ大きかったのは運動速度だけです(Kaya ら, 2021, Perceptual and Motor Skills)。
44名の試験でも、有意だったのはストループ課題の一部で、効果量は 0.12 から 1.13 までばらついています(Özeren ら, 2024, Journal of Bodywork and Movement Therapies、NCT05932004)。
認知課題と組み合わせる試み
ピラティスに認知課題を同時に行わせる「コグニサイズ」型の介入も試みられています。軽度認知障害のある11名に10週間・1回60分の複合課題ピラティスを行ったパイロット研究では、MoCA-K が 11.64 から 14.18 へ有意に上昇し、下肢・上肢の筋力も改善しました。一方、2分間足踏み、8フィートUp&Go、椅子座位前屈、背中かきテストには有意な変化がありませんでした(Cha ら, 2025, Physical Therapy Rehabilitation Science)。11名・対照群なしのパイロットです。
この方向性は、より大きな試験でも支持されています。軽度認知障害のある95名(平均72.12歳)を対象に、認知刺激のみの対照群と、認知刺激+運動の複合プログラム群を12週間比較した試験では、バランス、歩行、上下肢筋力、柔軟性、身体機能、認知機能、MoCA、言語流暢性、実行機能のすべてで複合群が有意に優れていました(Castellote-Caballero ら, 2024, BMC Medicine、NCT05503641)。
日本で行われた48名の試験でも、二重課題ベースの多要素運動+歩数計による歩行を12週間行った群で、記憶と実行機能が有意に改善し、機能的MRIで視覚的短期記憶に関連する脳領域(前頭前野を含む)の活動が減少——つまり神経効率が上がった——ことが示されています(Nishiguchi ら, 2015, Journal of the American Geriatrics Society)。
パーキンソン病の35名を対象とした試験では、マットピラティス群で短期に認知(p = 0.048)と混乱(p = 0.049)が、短期・長期の両方で抑うつ(p < 0.001)、不安(p = 0.033)、幸福感(p < 0.001)、孤独感(p = 0.017)が改善しました。対照群では抑うつと混乱が悪化しています(Moratelli ら, 2025, Aging & Mental Health)。
後期高齢のフレイル高齢者(75歳超)を対象に、週1回30分・8週間の改変ピラティスを行った研究では、遊脚の足部制御と動的バランスが改善した一方、実行機能に29%の改善傾向は見られたものの、認知機能に有意な効果はありませんでした(Sarashina ら, 2022, Applied Sciences)。
なお、ピラティスの心理的アウトカムを32試験1,264名で統合した解析では、調整前のモデルでは全体効果が有意にならず(p = 0.061)、アウトカムの種類で調整して初めて有意になりました(g = 0.393、p = 0.023)。異質性は I² = 91.7% です(Tsartsapakis ら, 2026, Sports)。

いま言えること
- 110名のピラティス試験で、言語流暢性と実行機能は改善したが、全般的認知機能(MMSE)は改善しなかった
- 585名・18か月の最も厳密な試験では、運動にもマインドフルネスにも効果がなかった。5つの副次アウトカムもすべて
- 一方、20の統合解析を集めたアンブレラレビューでは、心身系運動が最大の効果(d = 0.48)
- 効果量は解析によって 0.155 〜 0.48 と幅がある
- 75歳を超えた人と軽度認知障害のある人では、効果が最も小さい(0.08)
- 座りがちな人で効果が大きく(0.33)、すでに体力のある人では小さい(0.16)
- 期間が長いほど効果が小さい(短期 0.32 → 長期 0.09)
- 52名の試験では、ピラティスと有酸素運動のあいだに認知機能の差がなかった
- 認知課題を同時に行う複合プログラムでは、より広い改善が報告されている
- 有害事象の報告はない
小規模研究を集めると効果が出て、大規模で厳密な試験では出ない——これは出版バイアスや小規模研究効果の典型的なパターンです。
効果が出るとしても「小さい」水準です。MMSE で 0.65点、効果量 0.2 前後。
そして最も必要としている層——75歳超、軽度認知障害のある人——で、効果が最も小さい。
ピラティスが有酸素運動より優れているという根拠はありません。直接比較した試験では差がありませんでした。
実務的には、認知機能の維持を目的にピラティスを選ぶ根拠は弱いと言わざるをえません。ただし、効果が出た研究に共通する条件はあります——週60〜120分の中強度、週3〜4回、12週間程度。そして認知課題を同時に行う形式では、より広い改善が報告されています。
ピラティスを続ける理由は、筋力、柔軟性、バランス、気分など他にいくらでもあります。認知機能はそこに付随するかもしれない、という程度に見ておくのが妥当です。
